Troop 305 Forms

Following are a variety of downloadable forms for troop activities. All forms are PDF (Portable Document Format) files unless noted otherwise. You will need Adobe Acrobat Reader (a free application) in order to open and print these files.

Activity/Campout Permission Slip

All scouts must turn in a completed & signed permission slip for all activities other than troop meetings. Please note that some campout locations may require an additionl form.

Medical Form

All scouts must have a completed medical form on file. Medical forms will be provided to the facility for all long term camping (summer camp & high adventure). Be sure that all signuture areas are signed before turning into the troop.

Please note that BSA has changed the medical form. Now all scouts must renew their medical form annually using the form found here. (Previously the Class 2 form completed by a physician had to be completed every 36 months for scouts)

Completed forms should be returned to the Advancement Chair at a regular Troop meeting.

Driver Information Form

Any parent or guardian who plans to drive on a campout or other scout function should complete a driver information form and file it with the Advancement Chair in the troop office.

Service Hours

Any scout who works on service projects outside the troop should complete this form. Remember that all hours to count towards rank advancement should be approved by the scoutmaster BEFORE they are completed.
